They have a new affirmation form up. It might have been there for a while but I only seen the plain one. The new one has a place to print your name and is embellished with the state seal.

Also, the address on the permit application says ATTN: Firearms Unit as opposed to the Attn: License Unit on the webpage. I'm sure both will be fine but I'm tossing it out there.

Be careful and read the permit application, its not like buying a firearm you may have to answer yes to some questions ;)
